Support eligible Aboriginal and Torres Strait Islander clients by coordinating access to services and aids to assist in the self-management and treatment of diagnosed chronic disease as outlined in their General Practitioner care plan.

About CRAICCHS:

Cherbourg Regional Aboriginal and Islander Community Controlled Health Service (CRAICCHS) is a Aboriginal and Torres Strait Islander community-controlled organisation.

CRAICCHS objective is to provide access to holistic and client-centred quality primary health care and social and emotional wellbeing services to the Aboriginal and Torres Strait Islander communities in the South Burnett regions.

Key Duties:

The duties will be diverse, and although not limited to, will include:

  • Assist clients better understand their chronic health condition.
  • Support chronic disease self-management and assist with care plan compliance.
  • Coordinate activities and aids as outlined in the General practitioner’s care plan.
  • Provide appropriate clinical care consistent with skills and qualifications.
  • Utilise supplementary services funding to assist clients access planned services and aids.
  • Work with clients to improve their capacity to engage with the broader health system and access culturally appropriate health care services.
  • Advocate on a client’s behalf in health care settings.
  • Work in collaboration with other Integrated Team Care members to ensure continuous improvement for the client.
  • Provide education and awareness programmes through promotional events and displays in Cherbourg and other key sites in the South Burnett region.
  • Maintain accurate client records..
  • Complete timely reports both internally and to external stakeholders such as funding bodies.
  • Participate in formal training, peer support, professional guidance and mentoring.
  • As directed, complete other tasks within your scope of practice, skill level, competence and training.
  • Uphold privacy and confidentiality procedures and regulations.

Key Requirements:

The successful applicant will be able to demonstrate:

  • Relevant qualifications with a minimum Cert IV in Aboriginal and Torres Strait Islander Primary Health Care Practice.
  • Experience as a health professional in a primary health care setting.
  • An understanding of the general practice sector with the ability to work with mainstream primary care providers.
  • Case management experience.
  • Knowledge of chronic disease health conditions.
  • Experience in coordinating internal and external service providers.
  • Experience working with Aboriginal and Torres Strait Islander peoples and delivering appropriate health support and access to services.
  • An understanding of issues affecting Aboriginal and Torres Strait Islander peoples and their health, social and emotional wellbeing needs
  • Competent computer skills in the use of business technology, internet, MS Office, medical software (best Practice) and client management and quality management systems.
  • Excellent communication skills with the proven ability to effectively and sensitively engage with Aboriginal and Torres Strait Islander people, people with disabilities and those with diverse characteristics and life experiences to address client health needs and engage with health professionals.
  • The ability to self-manage while working effectively within a team-oriented environment.
  • The willingness to learn and complete ongoing professional development and on-the-job training.
  • Flexibility to work or travel within the region to meet the specific needs of the client or the programme.
  • Current Blue Card (Working with Children Check) or the ability to quickly acquire.
  • Current CPR and First Aid Certs or the willingness to obtain.
  • Current driver licence.
  • Documentary evidence to verify either vaccination against or are not susceptible to any of the following Vaccine Preventable Diseases: Hepatitis B; Measles, Mumps, Rubella (MMR); Varicella (chickenpox) and Pertussis (whooping cough).
